Payment Policies

Co-payments and deductibles are due at the time of service and are payable by cash, check, Mastercard, and Visa.

Our practice encourages you to become familiar with your health plan. To determine your insurance benefits and eligibility please contact your insurance carrier directly. Note that you are ultimately responsible for payment for all services that are not covered by your insurance plan.
